AOM 2mo - 2yo 80-90 mg/kg/day divided q8h x 10 days. Max 4g/day.
AOM 2-5yo 80-90 mg/kg/day divided q12h x 7 days. Max 4g/day.
AOM 2-5yo 80-90 mg/kg/day divided q8h x 7 days. Max 4g/day.
AOM ≥6yo 80-90 mg/kg/day divided q12h x 5-7 days. Max 4g/day.
AOM ≥6yo 80-90 mg/kg/day divided q8h x 5-7 days. Max 4g/day.
Pneumonia ≥3mo 90 mg/kg/day divided q8h x 7-10 days. Max 4g/day.
Acute bacterial rhinosinusitis, low-dose ≥2yo 45 mg/kg/day divided q12h x 5-10 days. Max 4g/day.
Acute bacterial rhinosinusitis, high-dose ≥2yo 80-90 mg/kg/day divided q8h x 7-10 days. Max 4g/day.
Group A streptococcal tonsillopharyngitis 3-18yo 50mg/kg/day divided q12h x 10 days. Max 1g/day.
UTI 40mg/kg/day divided q8h x 7 days. Max 1500mg/day.

Dose:
It is generally reasonable to limit individual doses to 500 mg for mild-moderate infections. Do not use this limit for pneumonia or severe infections.
