Loop diuretic. Inhibits Na-K-2Cl symporter in the thick ascending limb of the loop of Henle.
Indications
Third line for tx HTN
Most appropriate for the treatment of HTN that is complicated by
Reduced GFR (<30-40mL) OR
Volume overload syndromes (CHF or nephrotic syndrome)
A/E
Loops lose calcium
Severe hypokalemia
Electrolyte imbalances
Ototoxicity
Note
Can lower BP, but less effective in long term than Thiazides
Furosemide has a shorter duration of action than thiazides (less than 6 hours)
Torsemide is longer active (>12 hours)
