Toxicity
     Mechanism of toxicity unclear, possible etiologies include
          Phosphodiesterase inhibition
          Adenosine antagonism
          Stimulation of epinephrine release
     Often results from decreased clearance due to concurrent administration of
          Ciprofloxacin
          Erythromycin
     Sx
          CNS stimulation
               Headache
               Insomnia
          GI disturances
               Nausea
               Vomiting
          Cardiac toxicity
               Arrhythmia, drug-induced
