Angiotensin-Converting Enzyme Inhibitors
Note: consider discontinuation or dose reduction of diuretic when initiating an ACE inhibitor to avoid potential hypotension
- Perindopril (Coversyl)
- Ramipril (Altace)
- 2.5-20 mg PO daily; may consider divided dosing (BID) if necessary to maintain BP
- Lisinopril
- 5-40 mg PO daily (usually start at 10 mg if not on diuretic)
Angiotensin II Receptor Blockers
- Olmesartan (Olmetec)
- Candesartan (Atacand)
- 8-32 mg PO daily (may be divided over two doses)
Thiazide Diuretic
- Hydrochlorothiazide
- 12.5-50 mg PO daily (may be divided over two doses)
Thiazide-like Diuretic
Loop Diuretic
- Furosemide (Lasix)
- 10-40 mg PO BID (e.g. 8 am & 2 pm; titrate per patient)
Potassium-sparing Diuretic
- Spironolactone (Aldactone)
- 25-100 mg PO daily (may be divided over two doses)
Beta-blockers
- Metoprolol (Lopressor, Toprol XL)
- 50-450 mg PO daily (immediate release should be divided over two-three doses)
- Atenolol
- 25-100 mg PO daily (start 25-50 mg daily)
- Bisoprolol (Zebeta)
- 2.5-20 mg PO daily (start 2.5-5 mg daily)
Calcium channel blockers
- Dihydropyridine
- Nifedipine (Adalat XL)
- Amlodipine (Norvasc)
- Non-dihydropyridine
- Verapamil
- 80-160 mg PO TID (immediate release)
- Diltiazem (Cardizem, Tiazac)
- 180-480 mg PO daily (Cardizem CD) (start 180-240 mg daily)
- 120-540 mg PO daily (Tiazac, Taztia XT) (start 120-240 mg daily)
- 180-360 mg PO daily (Cardizem LA, Tiazac XC) (start 180-240 mg daily)
