Chest wall rigidity and glottic spasm are rare but classic unique side-effects of high doses of IV fentanyl given rapidly (generally >15 mcg/kg but has been observed at lower doses)
This may not be reliably antagonized by naloxone and may require neuromuscular blockade and intubation to enable adequate ventilation.
This complication is very rarely reported with the dosages of fentanyl used but can still happen.
