Hormone replacement therapy typical doses

Estrogen

  • Starting: 0.5 mg oral estradiol≈CE 0.300 mg oral tablet≈25 mcg transdermal 17-beta estradiol.
  • May increase to: 37.5 mcg transdermal 17-beta estradiol.
  • May increase to: 1 mg oral estradiol≈CE 0.625 mg oral tablet≈50 mcg transdermal 17-beta estradiol.
  • Young women after bilateral oophorectomy may require starting dose of up to 100 mcg transdermal 17-beta estradiol.

 

Progesterone

  • If the patient has a uterus, add progesterone (cyclically if perimenopausal) or continuously:
  • Micronized progesterone 200 mg/day cyclically (12 days per month) or 100 mg/day continuously.
